In order to tryout for any of the sports offered at Maitland, you MUST have an updated OCPS athletic physical on file at Aktivte.com. You will only need to upload page 4 of the EL2 forms.

Any other physicals will be denied and you will not be allowed to tryout until the correct paperwork has been uploaded.
